The 4th Grade Challenge Program is being offered at your school in partnership with Mosaic Life Care to help teach the importance of developing and participating in healthy habits. As part of that program, we would like to ask you a few simple questions around your current health habits and knowledge. All responses will be completely confidential. Thank you for your participation!
